Application Checklist

When you submit your Home Equity Loan application, please send or bring us copies of your:

1. Most recent paycheck stub with YTD information.
   
   
2. W-2 form for last year.
   
   
3. Personal and business income tax returns with all schedules for the last two years
   (Income tax returns are required only if you are self-employed or receive income from another source to be considered).
   
   
4. Last property tax notice and, if available, a recent appraisal.
   
   
5. Most recent mortgage statement(s) showing your balance(s) owed.
   
   
6. Mortgage payment book(s) and/or phone number of your mortgage holder.
   
   
7. Homeowner’s insurance policy and insurance company name, agent address, and phone number.
